In order to help you as best as possible, we would like to get to know you better. With a medical profile, we map out your health. It will allow the symptom assessment to better assess health complaints. In due course, we will also be able to assist you with tips and advice. You create a medical profile by answering some simple questions. All information is stored safely and can only be seen by you.
Take the following steps:
- On the main page of the Quin app, scroll to ‘Create medical profile’ and select ‘Start intake’.
- Answer the questions. Tap on ‘Next question’ to go to the next question. Go through all the questions and at the notification ‘Your medical profile has been successfully created’ select ‘Save’.
If you are unwilling or unable to answer
- Select ‘Skip question’.
- You can still answer this question at a later time.
If you wish to adjust your answer
- Scroll back up in the questionnaire.
- Tap on ‘Adjust’ to change an answer.
- Select ‘Save’ to confirm your answer.
If you wish to pause the questionnaire?
- Select ‘Pause’ at the top right of your screen. You will then return to the main screen.
- Your answers will be stored and you will be able to finish the questionnaire later.
Your medical profile has been created
After your medical profile has been created, your information will be safely stored in the Quin app. You will always be able to access your information, to review, change or delete it if necessary
Take the following steps:
- Select ‘My health’ at the bottom of the main page in the Quin app.
- Select ‘Medical profile’.
- You will now see an overview of the questions and the answers you gave.
- Would you like to change something? Tap on the question in the list and adjust your answer.
Would you like to check a health complaint? The symptom assessment will ask for access to the information in your medical profile. Based on this, the symptom assessment will be able to assess your complaint better. You only need to consent to this once.
